Overview

Superior Case Management is looking for an Independent Contractor to work with a close knit organization that is team oriented. This position is fee based. Independent Contractor must be available to work part-time and/or full-time hours. Independent Contractor must be responsible, reliable, be able to manage their time accordingly, be flexible, have a good working knowledge of auto no-fault, must have excellent writing skills, and be willing to travel through several counties outside of Metro Detroit. Independent Contractor will be able to work from home in between attending appointments.

Independent Contractor must have a Bachelor’s Degree in the Human Service Field such as Psychology, Social Work, Speech-Language Pathology, Occupational/Physical Therapy, Medical Case Management, Community Health Worker, Health Educator, Mental Health, etc. Independent Contractor must have prior experience being an Independent Contractor for a previous company.

Independent Contractor will have to spend 1-2 hours on the road, roundtrip depending upon where the appointment is located. This position is fee based. Independent Contractor will be expected to work 25-40 hrs per week. Superior Case Management will provide a lap top. A cell phone stipend will be offered in the the amount of 50.00. The hourly fee will be 32.50 per hour. This is a hybrid position. When you are not attending appointments with clients, you will be expected to work from home. Coming into the office to use the conference room is also an option as well. We must know ahead of time if the conference room needs to be used by any Independent Contractor and/or Employee.

As an In dependent Contractor you will be responsible for scheduling all appointments, which will include, but not be limited to all therapy appointments, doctor appointments, diagnostic testing appointments, etc. As an Independent Contractor you will be responsible for scheduling all transportation. As an Independent Contractor you will be responsible for any fees associated with trips in other parts of the state of Michigan that will require an overnight stay at a nearby hotel. You will be reimbursed for mileage associated with all medical appointments to and from our home office.

This position is fee based.


Job Description

  • Assessment: Evaluating a client’s needs; including physical, mental, behavioral, and social well-being, to create a personalized care plan.

  • Coordination: Connecting clients with the appropriate services and resources; such as healthcare providers, therapists, transportation, etc.

  • Advocacy: Representing the client’s best interests and ensuring they receive the care and support they need.

  • Monitoring and Evaluation: Tracking client’s progress, adjusting care plans as needed; and evaluating the effectiveness of services.

  • Communication: Maintaining open communication with clients, families, and service providers to ensure everyone is informed and working towards the same goal(s).

  • Documentation: Maintaining accurate and detailed records of client interactions, progress and services provided.

  • Crisis Intervention: Providing support and guidance during crises or emergencies.


RESPONSIBILITIES

  • Coordinate and attend appointments with providers such as doctors, therapists, diagnostic testing, transportation, home care services, DME needs, OT and home safety evaluations.

  • Secure durable medical equipment.

  • Contact and coordinate medications with pharmacy.

  • Research specialty programs for treatment of brain injury, pain management, spinal cord and or other medical issues.

  • Engage patients and their families, guardians and caregivers as part of the care team to advocacy, ongoing communication, service facilitation an identification of resources.

  • Advocate, educate, and coordinate care with interdisciplinary team members to maximize client objectives.

  • Must be able to adapt to using a software system for tracking and documentation purposes.

  • Request and obtained medical records and insurance resources.

  • Identify treatment options and community resources.

  • Communicate verbally and/or in writing with insurance carriers, attorneys to explain the need for services/advocacy of care.

  • Understand, identify, and coordinate care accordingly as it relates to coordinated insurance policies.

  • Candidate must be willing to travel across the state. Mileage reimbursement will be provided.

  • Must be able to provide your own liability insurance.

  • Professional Dress.

  • Must be responsible for car insurance.

  • Must have unlimited PIP.

  • Will receive mileage reimbursement according to current rate.

  • Must have liability insurance/workman's comp insurance.

  • Vehicle must be in good working condition.

  • Must turn in bi-weekly/monthly reports in order to receive proper reimbursement for services rendered. 


SPECIAL SKILLS, EXPERIENCE OR QUALIFICATIONS

  • Strong Communication: Written, Verbal and Non-Verbal communication is crucial for interacting with clients, families, physicians, insurance adjusters and attorneys both in person and on the telephone.

  • Problem Solving: Case Managers need to be able to identify and resolve issues that arise during the care process.  Ability to think outside the box.

  • Organization: Managing multiple cases and coordinating various services at the same time.  Organization of self, desk, vehicle.

  • Empathy and Compassion: Working with individuals facing challenges in many phases of their life requires empathy and a genuine desire to help.  Loving what you do; even on the bad days.

  • Punctuality: Being on time for client appointments.  Handing in paperwork on time.

  • Good Appearance & Hygiene: Dressed appropriately for a client’s appointment. No body odor (smelling like cigarettes, too much perfume/cologne). Hair, makeup and nails look appropriate.

  • Good Grammar: Accurate spelling.

  • Listening: Excellent listening skills with the ability to ask probing questions, understand concerns and extract relevant and sensitive information in a professional and caring manner.

  • Education: Knowledge of the law. 

  • Computer skills: Knowledgeable in the Microsoft Office Suite and Google Apps.

  • Transportation: Reliable transportation, automobile insurance, and a good driving records for the last 5 years.

  • Flexibility with schedule/time.

  • Ethical and discreet.

  • Responsible and trustworthy.
